Rural Healthcare Group Partners with Crossroads Medical Group to Expand Primary Care Access in Tennessee

RHG is excited to highlight our latest partner from 2023, Crossroads Medical Group (‘Crossroads’), a 6-location, 20-provider primary care medical group delivering high-quality primary care to rural and underserved communities in Middle Tennessee.

The Story Behind Crossroads Medical Group

Crossroads was founded in 2003 by Dr. Scott Jordan and Dr. Jeff Jenkins in White House, TN with the goal of bringing high-quality primary care to more rural areas where it was desperately needed. Shortly after founding, Crossroads expanded services with weekend and evening hours and promoted the concept “Medical Needs Aren’t Always Convenient, But We Are.”


As Dr. Jordan had experience in healthcare consulting, Crossroads continued to refine its clinic model to enhance the patient experience, provide timely care, and improve quality assurance protocols. With continued growth in population and patient needs, Crossroads expanded its footprint to adjacent communities to meet the growing demand. Dr. Jordan noted, “We wanted to eliminate the need for people in our communities to have to drive 20 miles to receive necessary care. With expanded access to care and a continued focus on patient experience, we believed that we would improve compliance and quality for the patients we served.”


Though they were growing into a larger practice, Crossroads strived to maintain the feel of a small town, community-focused group. This included accepting all insurances (Medicaid, Commercial, Medicare and Medicare Advantage), thereby increasing accessibility for all patients in the communities they served. Dr. Jenkins said, “We offer complete care, for all ages, treating the whole person and meeting our patients where they are.”


Over the years, Crossroads has successfully grown by adding locations, specialties, and providers in adjacent geographies throughout Middle Tennessee. Each year, Crossroads provides comprehensive primary care services to over 25,000 unique patients across all insurances through a network 6 convenient locations and nearly 20 providers.

The Partnership
